I do not think that you really need to be all that formal unless they ask you to be.. But just for your sake and to be organized I would come up with a small lesson plan or something along the lines of a syllabus. I used to tutor younger children so it helps to be a little organized so you are able to get through the material easier. As for the materials, that should usually be provided but it doesn’t hurt to bring an extra pen. There is usually a department of the main office with someone oyu can talk to about becoming a tutor.
